Support to find housing and access essential health services

The Assistance with Care and Housing program provides practical support to find affordable housing, and provides referrals to health and social services.

If you are aged 50 or over, are on a low income and are homeless or at risk of homelessness, the Assistance with Care and Housing program can support you.

Access help to find appropriate, sustainable and affordable housing

Finding the right accommodation for your needs can be difficult enough, but submitting paperwork, organising arrangements to relocate and accessing relevant support and loans can be overwhelming.

Our Assistance with Care and Housing team can provide you with assistance to:

• locate appropriate properties for you
• complete and submit paperwork required to apply for a tenancy or social housing
• organise transport to view properties
• understand the requirements of a new tenancy
• access relevant loans
• arrange relocation (book movers, etc)
• advocate for your needs
• access health, community care, welfare and social support services through referrals
• maintain or build local support networks in your community

Support can be provided in a variety of settings including within your own home, or at a local centre or clinic in your community.

Individuals, carers, and family members can access the Assistance with Care and Housing program by calling one of the locations listed below.

Community service organisations and health professionals can also make referrals into the Assistance with Care and Housing program by calling us.

Fees

There is no charge to access this service.

This service is funded by the Commonwealth Government Department of Health.
