Access support to stay independent in your home and community

If you are in need of support to live independently in your home and community, we can link you with essential support services and advocate on your behalf.

We support you to stay independent and safe

We can visit you in your home, talk over the phone or work out what’s best for you. We will try our best to understand the specific barriers you face and liaise with services to ensure that your needs are met.

Our team advocates for, and supports people who are:

  • financially and/or socially disadvantaged
  • living with dementia or cognitive impairment
  • a veteran
  • living with an illness or disability
  • experiencing homelessness or at risk of homelessness
  • a care leaver
  • living in a rural or remote area
  • culturally or linguistically diverse
  • Aboriginal or Torres Strait Islander people
  • identify as LGBTQIA+

If you are aged 65 or over, we can:

  • link you into services to help enable safe and independent living in your home
  • provide information about the My Aged Care process
  • support you to apply for My Aged Care
  • assist you with completing paperwork and/or online applications
  • refer you to services, support you at appointments, and help you to understand service fees and costs (if applicable)
  • support you to access government payments and services (e.g. Centrelink)

If you are under 65 years of age, we can:

  • link you with services that are available to keep you living in your home as safely and independently as possible
  • provide information about Living at Home Assessments and/or the NDIS process
  • support you to gather evidence for NDIS and Centrelink claims
  • assist you with completing paperwork and/or online applications
  • refer you to services that assist you to get help around your home
  • help you to understand service fees and costs (if applicable)

Who is this service for?

  • Individuals, carers, and family members living in the City of Ballarat, Moorabool Shire, Hepburn Shire and Golden Plains Shire
  • Anyone with dementia or cognitive impairment
  • People located in a rural or remote area
  • Veterans
  • People experiencing homelessness or who are at risk of homelessness
  • Anyone financially and/or socially disadvantaged
  • LGBTIQA+ community
  • Anyone on carer’s leave
  • Aboriginal or Torres Strait Islander people
  • People from a culturally or linguistically diverse background

Fees

Free.

This service is funded by the Victorian State Government Department of Health.
