Access support to get back on track

Your Case Manager will work with you to:

understand and discuss your specific needs
help you access secure and sustainable housing
provide referrals to appropriate specialist and community supports
advocate for you and your wellbeing on your behalf

Who is this service for?

Referrals into this program are only accepted from Ballarat Health Services’ Adult Acute mental health inpatient service.
People aged 18 and over who are exiting Ballarat Health Services’ Adult Acute mental health inpatient service and are at risk of homelessness.

Fees

Free.

This service is funded by the Victorian State Government Department of Families, Fairness and Housing.
