The Case Manager at Peplow House is responsible for the intake, assessment, and delivery of intensive case management for individuals entering crisis accommodation. Using a strengths-based, trauma-informed approach, the Case Manager provides high-quality services to homeless men aged 18 to 64. The role involves collaborating with clients in a friendly, professional manner to develop and implement client-led case plans, addressing barriers and supporting the achievement of life outcomes identified by the clients themselves.

The SETS program works closely with Humanitarian Entrants in their first 5 years of arrival in Australia to address their settlement needs, to improve social participation, economic well-being, independence, personal well-being and community connectedness. The position is responsible for ensuring the effective and efficient coordination of SETS, Settle Well and Job Readiness. The position is responsible for coordinating and monitoring caseloads, including providing support, guidance and supervision to staff to ensure positive outcomes. The Team Leader, Settlement, Engagement and Transition Support, is responsible for overseeing the provision of settlement support to newly arrived people, especially Humanitarian Entrants and Refugees, who have arrived in Australia within the last five years. Support includes low and medium intensity case management, group based learning and community capacity building.

The Family Services program aims to promote strong families with children who ae safe, healthy, resilient and thriving; and parents and carers who are supported to create safe and nurturing home environments. Along this continuum of support, the Family Services program provides responsive, flexible and sustainable support to children and families with the intensity that is required at the point when it is needed. The Team Leader, Family Services works with the other Team Leaders in the program across CatholicCare Victoria. Team Leaders provide support and supervision to practitioners engaged with children, young people and parents/caregivers in a child focused, empowering and meaningful way. Using a Relational Approach, practitioners work with families to Connect; Understand; Link-Up; Build and Empower; and Create Opportunities. Close working relationships with other stakeholders, including Child Protection, are built to ensure that children can remain in, or return to families safely. Team Leaders and Practitioners work collaboratively and collectively with service providers. Delivery of rapid, flexible, culturally safe and intensive service when and where families need them and are delivered through a lead practitioner model supported by a care team approach.

The Family Relationship Centre (FRC) provides Family and Property Dispute Resolution services. Services may be provided in the FRC or other locations across CatholicCare Victoria according to the needs of clients and communities. The Family Dispute Resolution program aims to assist separating parents and families to reach agreement on parenting arrangements which are in the best interests of children. Property Dispute Resolution assists separating couples to reach an agreement in relation to their property pool, including debt. Both Family and Property Dispute Resolution supports separating families to resolve disputes as an alternative to court. The Family Relationship Advisor role is to provide high quality intake, assessment, support and referral advice to clients of our Family Relationship Centre.

The Family Services program provides a continuum of support to families in the community, ranging from supporting them in their parenting role, to working intensively with families under increased pressure whose children are at risk of placement in care services, or have been placed in care and are able to be reunified with family in a short timeline, through the Victorian and Aboriginal Family Preservation and Reunification Response (FPRR).

Practitioner, Family Preservation Reunification Response

The FPRR Practitioner will provide outreach wraparound services to families where children’s ages range from unborn to adolescents. Working flexibly, as part of the team they will report to the FPRR Practice Team Leader. Areas of clinical focus include risk assessment and safety planning and practitioners will strive to empower caregivers to help their children and young people live safely in the community, attain school/vocational goals, and develop prosocial behaviours.

CatholicCare Victoria is a Settlement Engagement and Transition Support (SETS) provider, funded by the Department of Home Affairs. The SETS program works closely with Humanitarian Entrants, other eligible permanent migrants and their communities to address their settlement needs, in order to improve social participation, economic well-being, independence, personal well-being and community connectedness. Another purpose of the SETS Program is to build the capacity of small and/or ethno-specific organisations to better support their local communities. CatholicCare operates SETS from Dandenong, Wyndham Vale and Epping sites. The Settlement Case Manager is responsible for providing settlement support to newly arrived people, especially Humanitarian Entrants and Refugees. Support includes low and medium intensity case management, group based learning and individual capacity building.
