Assistance for residents and proprietors of Supported Residential Services
The Supporting Connections service supports both residents and proprietors (owners) of Supported Residential Services (SRS).
This service improves residents’ access to local health and social services, and builds the capacity of SRS proprietors and staff in identifying, planning for, and addressing resident needs.

Improve your wellbeing and social connectedness
If you are living in accommodation with Supported Residential Services and require additional support to improve health and wellbeing, our Supporting Connections program can refer you to relevant support services.
We can also help to increase your participation in social activities – reducing isolation and helping you to get involved in your community.
Our Supporting Connections staff can support residents to:
- identify unmet health or social needs
- access local health and social services through referrals
- improve outcomes for social and health needs
- increase social participation
Proprietors, access support to improve care and planning
Our Supporting Connections staff can help SRS proprietors to:
- identify resident needs
- improve care planning
- better manage challenging behaviours
- strengthen relationships with local services
- build skills and knowledge to support residents and build residents’ confidence and independence

SRS proprietors can access or make referrals to this service by calling the location listed below.
Interpreter services can be arranged if required.
Fees
There is no charge to access this service.
This service is funded by the Department of Health and Human Services (Victorian State Government).
