Support to access aged care and community services

Advanced care finder supports people to access aged care services or other supports in their local community.

If you are eligible for aged care services, but need extra support to navigate the aged care system, our Advanced care finder team can support you.

Connect into and learn about local services

Our Advanced care finder team will work with you to discuss your needs, and will help you to understand the different types of aged care supports available.

Our goal is to ensure you can make an informed choice about the providers or services that will be most suitable for you.

Our team can also help you to access supports and services including:

  • My Aged Care, including online services
  • Navigate assessments
  • Build capacity towards independent living
  • Connect you with local health services, mental health support services, social support services
  • Connect you with housing and homelessness support services, services that support independent living
  • Connect you with drug and alcohol support services, community groups and services that promote wellbeing
    and inclusion.

The Advanced care finder program provides flexible, safe and tailored support to suit your needs.

Our team can provide support to you at home, in your community, in one of our offices or even over the phone.

We listen to your story and experiences, so that we can understand your needs and provide you with the best support possible.

To ensure our service is accessible to all, we can also provide access to:

  • Translation services
  • Auslan Connection
  • Indigenous interpreting services
  • National Relay Services.


How do I access this service?

Advanced care finder can be accessed by:

  • people who are eligible for government funded aged care services 
  • people who do not have existing supports available to assist with navigating services
  • require intensive support to navigate and access aged care services

We also welcome referrals from all services, including self referrals from people living in Barwon, Central Highlands, South West and Wimmera regions.

Fees

There is no charge to access this service.

This program is supported by Western Victoria Primary Health Network under the Australian Government’s Primary Health Networks Program.
